PREPARING FOR YOUR FIRST VISIT

If this is your first time visiting our practice, please arrive 10-15 minutes prior to your appointment to allow ample time to complete client intake and consent forms for each of your scheduled procedures.

CLIENT RESPONSIBILITY

Client is responsible for the payment of services rendered. Payment is expected at the time of service for all charges towards current visit, as well as previous balances (i.e: late fees, no-show fees). We accept cash, Visa, Master Card, American Express, Discover and Care Credit. Gratuity for services are never expected, but always appreciated.

LATE POLICY

We ask that clients please arrive 10-15 minutes early so that we can start your service on time. A late arrival may reduce your appointment time out of respect to the clients scheduled after you. We will make every effort to accommodate your full appointment, but this is not always possible based on our other scheduled appointments.

CANCELLATION & RESCHEDULING POLICY

The most valuable thing that you can give someone is your time, and we want to be respectful of everyone’s time. In the event you cancel less than 24 hours before your appointment or do not show, you will be charged $50 for any appointments allotted for 15-45 minutes, and $100 for any appointment 1 hour or longer. The fee must be collected before you receive your future services. In the event that you no show more than three times, you will need to pay a $100 deposit in order to schedule any future appointments.

SICK POLICY

In an effort to maintain a healthy working environment for our staff, we ask that if you are sick (which includes a cold, fever, chills, or other flu-like symptoms) that you reschedule your appointment. If you arrive for your appointment with symptoms of an illness, you will be asked to reschedule your appointment. This is for your well-being as well as the health of our employees and other clients. If you do need to cancel your appointment, please call us as soon as possible and we will be happy to reschedule you for a time when you are feeling better.

REFUNDS

Pre-paid services are not subject to refund unless approved by a manager. If you do not wish to complete your pre-paid service, we can give you a credit with Uptown Medical Aesthetics that can be used towards another service or product.

We have carefully selected SkinMedica, Obagi Medical, and Jan Marini products because we trust their medical research. We do not offer refunds on open products. If you have a reaction or a malfunctioning product, we will issue you a credit of we will replace the malfunctioning item.

PAYMENT

We will now be collecting a credit card at the time of appointment booking. In the event of a no-show or an appointment cancelled passed the 24-hour window, your card will automatically be charged. We understand that there will be certain unforeseen circumstances and will take account into those! But, if you look on the bright side, it also ensures a CONTACTLESS PAYMENT option!

PREPAID SERVICES

Prepaid Services must be used within ONE YEAR of purchase date.

CHILDREN

We ask that you do not bring your child/children to your appointment. Due to the sensitive nature of the procedures we are providing, the area could be dangerous to a child. There are needles, lasers, numbing agents, and other sharp objects in the office that can harm a child.
